About Hmong Health Collaborative

The Hmong Health Collaborative (HHC) operates a partnership health advocacy project in Central and Northern California. Since 2004, we have provided direct individual assistance to Hmong health care consumers, conducted research on the health care needs and options for Hmong families and successfully advocated for policies that increase culturally and linguistically competent care for the Hmong.  The California Endowment made our collaboration possible and supported our work from its inception in 2004 through fall 2010. The HHC currently works with Pacific Clinics as a sub-contractor on the California Reducing Health Disparities Project (CRDP), funded by the California Department of Mental Health.

The HHC is a partnership between non-profit organizations located in Fresno, Merced, Modesto, Stockton and Sacramento Counties that are working to improve health care access and quality for Hmong families. The partner agencies include seven community based organizations. The HHC members provide a variety of services to the Hmong community including interpretation, case management, citizenship classes and employment services.  We bring extensive experience in community outreach, education, advocacy and training on health care access concerns on behalf on the Hmong families. The Collaborative envisions that its work will result in empowerment of Hmong families and individuals to access quality health care and to lead health lives.
